  • Who is Eligible?

    All 1099 and full-time W2 employees of the organization working at least 20 hrs per week

  • Can I change my elections at any time?

    • This is an IRS qualified plan and therefore is subject to rules for changing your elections. You must have a qualified status change to change your elections.  You will be given the opportunity to make changes each year during annual enrollment.
    • You can cancel your participation in the plan at any time, should you want to transition to another health plan or a different type of an organization.
    • You must provide a 30 day notice via email. 
  • Is there a cost to enroll?

    • There is an initial enrollment fee of $500 plus a monthly fee of $15 per participant (ee only not spouse of dependents). This is reviewed annually prior to open enrollment.
    • There is an annual enrollment fee of $500 /per participant upon renewal of the plan year and each subsequent year along with a monthly fee. 
  • What type of plan is it?

    The Group Health Plan offers:

    • Dental
    • Vision
    • Life; and
    • Disability

    Rates are guaranteed for each enrollment year.
    Calendar Plan Year – effective January 1 – December 31.
