Plan Participation Fees:

Initial Benefit Plan Enrollment Fee:
  • $350 per plan participant/employee
  • $10 per month
 Annual Enrollment Fees:
  • $250 annually in November of each year
  • $10 per month thereafter through the end of the plan year

If you MUST cancel/change coverage, it must be due to a qualifying event. A 30 day written notice is required, failure to do so will result in premium taken. All elections made are for that particular year (ie 2011, 2012). Any drafts returned will result in an additional $45 fee.
